Student Item Authorization Form

Parents & Guardians: Please Complete This Form Before Your Student’s Item Is Engraved

Is your student bringing us a calculator, laptop, tablet, tumbler, school supply, or another personally owned item to personalize?

Before iMaK Design can engrave or customize an item supplied by a student or parent, a parent or legal guardian must complete and sign the authorization form below.

This form helps make sure you understand the customization process and have approved the item, design, and permanent alteration of your student’s property before we begin.

Why is an authorization required?

Laser engraving and other customization methods permanently alter an item. While we take great care with every project, materials, coatings, finishes, and electronics can react differently during the customization process.

Because you are providing an item that was not purchased from iMaK Design, there is always some risk involved.

Before sending an item with your student, please keep the following in mind:

• Do not send irreplaceable or highly sentimental items.

• For expensive items—especially graphing calculators, laptops, tablets, and electronics—we strongly recommend having a backup or replacement available whenever possible.

• Please double-check your student’s name, spelling, design, wording, and requested placement before approving the customization.

• Please back up important information on electronic devices before they are submitted.

• If an item already has scratches, cracks, dents, damaged coatings, or other existing damage, please disclose that information on the form.

• School-issued devices, calculators, electronics, or other property cannot be permanently customized without permission from the school.

• iMaK Design may decline an item if we cannot safely identify or customize its material.

If Your Student Is Bringing the Item to Us

You do not need to be present when your student drops off the item as long as this form has been completed and signed beforehand.

This includes students using our Estero High School pickup option. If your student is giving an item to Ben at school to bring to iMaK Design, please submit this authorization before your student gives the item to him.

The authorization form must be on file before customization can begin.

Complete the Parent/Guardian Authorization

• Enter the parent or legal guardian’s information

• Provide information about the student and item being submitted

• Carefully review the authorization and risk acknowledgment

• Verify the requested personalization and spelling

• Complete all required initials and acknowledgments

• Electronically sign and submit the form

Submitting this form authorizes iMaK Design to perform the requested customization, but it does not guarantee that an item can be engraved. We may need to inspect the item first to confirm that its material and construction are suitable for our equipment.
